Section 01
Used to open a new NPS account through a Point of Presence or nodal office. eNPS replaces most of these in the online flow.
The standard physical registration form used by All Citizen and Corporate subscribers when opening an NPS account through a PoP. Captures personal details, KYC, scheme preference, and nominee information.
All Citizens forms (Protean CRA)Registration form used by Government sector subscribers, processed through the relevant nodal office (DDO, PAO, or PrAO). Equivalent to CSRF in function but follows the Government sector workflow.
Government forms (Protean CRA)Used by a parent or guardian to open an NPS Vatsalya account in the name of a minor. Captures the guardian's KYC, the minor's identifying details, and nominee or beneficiary instructions.
All Citizens forms (Protean CRA)Section 02
Used to update personal details, nominees, scheme preferences, or to activate Tier II after the Tier I account is live.
Used to update name, address, contact details, bank details, or other personal information on the PRAN account. Most fields can also be updated through the CRA portal or app.
All Citizens forms (Protean CRA)Used to change Pension Fund Manager, switch between Active and Auto Choice, modify asset allocation, or switch lifecycle fund. Frequency caps apply: PFM once per financial year, scheme preference up to four times per financial year.
All Citizens forms (Protean CRA)Used to activate a Tier II account against an existing Tier I PRAN. Tier II offers no lock-in or tax benefit but uses the same PFM, CRA, and PoP infrastructure as Tier I.

Used for permitted partial withdrawals from Tier I before exit, subject to PFRDA's purpose and frequency limits.
Used by Non-Government sector subscribers to request a partial withdrawal from Tier I for permitted purposes such as higher education, marriage, treatment of specified illnesses, home purchase, or skill development.
All Citizens forms (Protean CRA)Used to withdraw from a Tier II account. Tier II withdrawals do not require a permitted purpose since Tier II has no lock-in.

Used at superannuation or voluntary exit. Government and Non-Government sectors use separate exit forms because the underlying exit rules differ.
Used by Central and State Government subscribers exiting NPS on superannuation. Captures lump sum and annuity instructions and is processed through the nodal office and CRA.
Government forms (Protean CRA)Used by All Citizen and Corporate subscribers exiting NPS at age 60 or above, or under permitted premature exit. Captures lump sum, annuity, and Annuity Service Provider selection.
All Citizens forms (Protean CRA)Submitted alongside the exit form. Records the selected ASP, the annuity variant (life annuity, joint life, return of purchase price, family income), and payout frequency.

Used when the subscriber passes away, by the registered nominee or legal heir. The forms differ from the standard exit set because they record claimant identity and documentation.
Filed by the nominee or legal heir of a deceased Government sector subscriber. Routes through the nodal office and CRA, with prescribed supporting documents.
Government forms (Protean CRA)Filed by the nominee or legal heir of a deceased Non-Government sector subscriber, typically routed through the associated PoP. Requires the death certificate, claimant KYC, and applicable supporting documents.
All Citizens forms (Protean CRA)Physical grievance form used at a PoP or nodal office to log a complaint into the Central Grievance Management System when online channels are not used.
